If your participation in the program was declined, it may be due to one of the following reasons.
The number one reason for being declined is you applied for our program though a group that requires active medical insurance coverage and we were not able to verify your coverage. This is usually because of a typo on the insurance number or group number entered on the enrollment. This would be the case if the number includes a O (capital letter) but a 0 (number zero) is used instead.
If the class that your enrollment was for has not begun, we can check your insurance information manually to see if we can verify your coverage and get your enrollment approved.
We will need all of your pertinent information including full name, insurance numbers including member and group number and your date of birth. You can also submit a picture of your insurance card to verify your coverage. Send this information to our Member Care dept by clicking HERE.
Other reasons your enrollment might have been declined:
- You are pregnant.
- You are currently being treated for an eating disorder.
- Your health care provider has advised against participation in a weight loss program.
- You have a BMI beneath the recommended range for weight loss. (check your BMI here)
If you are enrolling through an employer, there may be additional criteria for participation. Your employer might limit participation based on:
- Employment status.
- BMI and/or risk factors for Metabolic Syndrome
- Number of spot available
Check your employer enrollment page for more information regarding your eligibility requirements. Go to the enrollment page and scroll down a bit to Eligibility with a + sign. Click that and the details for your group will display.
